Q: Do the cables come with it?

Mine came with a power supply and a short TOSLink (digital optical) cable. It does not come with RCA cables for output.

Q: I have a tv and 5.1 speakers does it work with that equipment?

Only if all speakers are internally powered (they have to be plugged into a power outlet). If not you have to by a Preamp, and the you won’t need this

Q: Will this work from converting sound from my tv with optical to my rca surround sound?

That’s exactly what I did and it works pretty well. Just have to make sure it’s set on 5.1 output instead of 2.1 for the best quality.
