5 volts is the USB-A input side. The output is on the barrel plug and is 12V.

Q: I have a 5v/3a power bank that I'm planning to use to power up a 12v 1.5a monitor. Will this work?

The max output current for 12V is 1 amp. Your monitor requires 1.5 amps so technically the draw over the rating of the adapter and will not work.

Q: Could this be used for audio equipment as a 12v trigger to activate (an amplifier) from another device that has USB but no trigger output?

Probably will work. I used a 5v usb connector in my car to feed this device. It gave me a 12 v output to supply a radar detector.
