This is a non-harnessed high back booster seat. The vehicle lap and shoulder belt must be used for the booster occupant.

Q: Does this booster fit well in the middle seat?

I don't think it would, it's pretty wide and takes up the width of a regular rear seat.

Q: can this be used by a person with autism or special needs ...?

That is a personal preference. As long as the child is mature enough to sit upright in the booster seat and not put the belts under their arm or behind their back, this would be a great option.
