Q: Is it comfortable to keep in your back pocket like a regular wallet?

It is harder than a regular leather wallet and I always carry mine in a front pocket. I have not noticed any discomfort in a front pocket. I would imagine that any discomfort from having it in your back pocket would come when you sit down, just like a regular wallet. There shouldn't be any discomfort while standing up though.

Q: Does it hold just one card without it falling out?

Yes it does, the rest of the space where other cards will go might fill up with pocket lint but it will hold just one card.
