Do you need original game to play this?

I bought this without owning the original game and it was still super fun. I don't think you need the original in order to enjoy the expansion.

When you draw a card, do you keep it or put it back in the pile and only keep it if it tells you to?

You put the card back in the pile after you draw and read it, unless the card specifically tells you to keep it.
