Q: how do I use it? I only got the warning pamphlet

The piece with the bunny ears is a cup, you can drink from there. The cream/grey part has a button that says PUSH, if you press it, the liquid comes out when you tilt the bottle, when you are done pouring the liquid you press the PUSH button again to close it. That same piece is a screw cap, you can take it off to fill the bottle and clean it.

Q: is this dishwasher safe? including the lid with the ears?

This is not dishwasher safe. We hand wash it after each use.

Q: Can you put this in the dish washer?

yes but be sure to take it out as soon as it dries to avoid falling
