Q: What are the actual measurements of the insert section ... particularly the height?

Across the top it's 24.5" the height is 19" and it's depth is 5.25". Keep in mind if you're recessing it that the vents for the air in take for the heater are on the top and upper back panel. So you'll need to leave some room for it to pull air in.

Q: Where does the heat blow out of? Is it the front top?

Yes, it blows out of the front vent, the top one. The bottom cover isn't a vent at all it's just a hinged door that hides the controls. The intake is on the back side up top.

Q: are the measurements of the insert section or the black decorative outer lip?

For the black decorative lip. I installed it in a 28in opening it had 1/4in overlap on each side and it looks good.
