The holes on the sides are 2 millimeter then on the bottom is 3 millimeter

Q: Does this have a hanging chain

It does not have a chain; but it has a fold-down handle that is a little less than 2" long. It snaps into place whether down or up. NB: This is not a tea strainer. It's taller than a soda can and more than an inch wider. (Some buyers who don't read details are shocked by its size.)

Q: Is this a very fine mesh or will small pieces of tea leaves escape the container, please?

Small tea leaves will escape. You will need to strain your tea again. I use a coffee filter to strain mine.

Q: Will this fit through a 4 inch diameter opening? I'm afraid the latches will hit.

It definitely will NOT fit. The hinge and latch increase the diameter to 4.5 inches.
