Do you have to use a key to open from outside

Once you unlock it, it will remain unlocked, you have to use the key only on the outside to lock and unlock it. You can lock and unlock from inside using only the thumb turn.

If the lock doeesn't fit my door, can I return it and get a refund?

You should always take your existing lock out and measure it first, before you purchase one. It needs to be exact.

Can you confirm the 'T' means the inside measurement? I need a lock with 40mm outside 50mm inside

"T" simply means THUMB TURN cylinder lock , from left is always internal measurement, near the tumb turn handle, otherwise stated. Therefore T50mm/40mm means the thumb turn is on the inside which is 50mm and the outside is 40mm. If the "T" (thumb turn) is to the LEFT it is on the inside and to the RIGHT it is on the outside. You should choose T50mm/40, hope this helps.
