Works great if older child is willing
The Dentist gave us a size small which is all she had but my daughter refused to wear it and could get her thumb our of it. A few years have passed and we sat her down and said it was time. She said she was ready. By now she was almost 7 years old. The size small no longer fit. Since she was almost 7 and tall for her age we ordered the large. It works because she is willing to wear the thing. If she wants she can still get her thumb out even with the improved design. The medium probably would have worked better and she may not have been able to get it off. Don't know. But because she was willing to wear it and use it as a tool to help her stop it is working great. She wore it for the first month anytime she was not at school. Then the second month only at night. Then we decided to see how she would do. She did great for a few weeks until we had a couple of late nights and she got sick and then we started catching the thumb back in her mouth. So we are back to having her wear it at night but she is willing. I think that is the key!Wish I would have known about this when she was young and would not be able to get it off.

Ineffective
I bought this product to use with my three year old daughter, who was an avid thumb sucker. She agreed that she wanted to stop, but when it came time to actually do it she needed a little help, so we got her one of these. The basic problem is that it is far too easy to circumvent by just popping the thumb out from under the little tube it fits into. When the temptation would get strong for her, she would do exactly that. She could also put the thumb back in the tube in a matter of a couple of seconds, but when it goes in there wet it stays moist for a long time and is somewhat unsanitary.I gave it three stars because it worked fairly well during the day to remind her not to suck her thumb, but it failed utterly at night when she wanted to use it to get to sleep. I ended up using a small piece of athletic tape to serve as a more effective reminder on her thumb, which seems to be working much better and costs a few cents every night. If I were doing this again, I would not buy this product and would just go with the tape from the outset. It is much more comfortable for her, gives her much better use of the thumb for other things, and allows her skin to breathe.

Good for absentminded thumb sucking
We use this on our 4 year old at night time, since she sucks her thumb in her sleep. We used the finger version of this product on her older sister a few years ago and it worked well, so we ordered this thumb version with high hopes. I don't remember the product being this pricey before though, wish it were a little less expensive. It works as intended, it is just a little too snug on our 4 year old's thumb joint. It leaves an impression in her skin and leaves it red and irritated for a while in the morning after wearing it overnight. We tried just loosening the attachment, but it still leaves deep indentations into her hand. So we now just put a band aid or some sort of skin barrier on before she puts on the thumb guard, and so far it is working a little better. If you have a 4 year old who is greater than the 50th percentile in growth, I would recommend ordering a size larger. Otherwise, the product does what it is supposed to do- stops her from sucking her thumb!

Good in concept but does not work
I bought this for my son who is approx 39 months old. I didn't love his thumbsucking (which he has done forever) but was willing to give him until 5-6 years old to stop. He does it so often and unconsciously that his teacher is telling me that it interferes with his work because he doesn't want to take his thumb out to work. So I bought this hoping it could stop his habit. We've only had the thing for maybe 1 week but we keep having the problem of him taking his thumb out whenever he wants to to suck on it, then putting it back when he knows we are nearby! And I know the manufacturers say "You must be installing it wrong..." (paraphrasing!) - we are not putting it on wrong. We are following all directions, looping it on everything we need to, and the bracelet is snug without being TOO tight. I've tried various things from putting bandaids in his thumb FIRST before putting the guard on to deter him. I'm not sure if this is the problem (and if this guard really does work as it should then this should be the only possibility) but my son is small for his age. Maybe around 35 percentile in terms of height and weight. He's not a shrimp, he's just a bit smaller than your average 3 year old. But the thumb portion actually fits quite well - it's not huge on him- and the whole thing "fits" relatively well, but he is still able to wriggle it out. If he can take it out whenever he wants, this will never work.I'm not sure what will happen over the next few weeks, but it doesn't look promising.......
