Q: does it come with or need special mattress or sheets or does standard twin work?

This does not come with a mattress. You will need to use a crib sized mattress or another name for the mattress is "toddler" size. A twin size mattress is much to big for this bed frame. This does not come with sheets.I think it was suppose to come with some sort of support area for the bottom of your mattress, something like a thin piece of plastic, but I didn't receive it with my order. I just put our crib mattress straight in the ship, and it's fine. ((You'll need batteries for the light fixtures.)) I bought this for my 3 1/2 year old son, and he really enjoys it. (And so does his 6 year old sister and all the little kids that come over!)Hope this answer helps. Feel free to ask me more! Thanks :)

Since your son is only 18 months old, he can use it at least for another 3 years (assuming he is in the normal or 50 percentile for his height).
