Q: Is this a handle and trim replacement for a moen valve ?

Moen T3691 is trim only. You must purchase the actual Moentrol valve body separately. Note that Moen has two types of tub/shower valves. The Positemp series only rotates. The flow is not adjustable, just the temperature when you rotate the knob or handle. The other, the Moentrol, which is what this trim is for, both pulls out to control flow amount of water and rotates to control temperature. The "ORB" after the number indicates the finish is oil-rubbed bronze.
