Q: My nipple is about 13.5 which size do I need 15 or 17?

I believe they said do 4-5 up so I’d try the 17 possibly. Each person is different. I Measure at a 15 and the 15 is too small so I use a 17 mm. 19 is too big for me.

Q: Are these safe for sanitizing in the microwave?

From my experience these types of items should only be sanitized by leaving in boiling water for no more than 5 minutes, or in your dishwasher if it has a sanitizing feature.
