It cushions my ankle nicely and is so soft not at all harsh or uncomfortable against my skin. It gets sufficiently cold for me and stays cold for about the amount of time typically recommended for icing an injury.I personally wear it over a thin sock. This helps keep the sleeve clean and also prevents the cold from coming into direct contact with my skin. I have a circulatory condition and am quite sensitive to cold, so this makes it much more comfortable for me. If you’re less sensitive, you could certainly wear it directly on your skin.I’m so happy with this purchase that I’m considering buying a second one so I can keep one in the freezer while using the other.Even after my ankle has fully healed, I think I’ll continue using these sleeves. I can see them being helpful for my plantar fasciitis, bone spurs, or simply soothing tired feet after a long day. I may even try using them on my wrists when my carpal tunnel flares up.Overall, this has been an excellent purchase. Fits perfectly on the leg. I used it in both hot and cold modes. In both settings, it doesn't retain the heat or cold for very long. Cold lasts around 10 minutes, heat 7-8 minutes.The wrap is very comfortable and fits excellently on the leg. Excellent quality.

Fits like a sock and so hugs the ankle and shin nicely. The only drawback is the cooling gel thaws very quickly giving you only minutes of relief.
