Q: Can this treat both sides or do i need 2?

It would treat either side. However, it didn't work for me. Good luck.

Q: Where does the inner tube sewn in go, front or back

It goes round the back so it sits on the nerve that transmits the pain, it works brilliantly for me so please give it a go. The tube should be at the top near where the knee bends and the v part at the bottom do it tight so it stays in place, give it at least a day the first time you use it, and keep it on in bed. After that as soon as pain starts use it straight away. It doesn't work for everyone but anything is worth trying to help the pain as I found out.
