Yes, they rotate very slowly at the same rate the Earth rotates. Or alternatively, there are two buttons that allow you to speed the rotation up a bit, both clockwise and anti-clockwise.

I've recently purchased this for our 4-year-old and can confirm that the images do rotate (four speeds available). It is reasonably quiet, you can hear a little bit of noise while it rotates - you can also opt for it to stand still. I think you can buy a separate adaptor, just a standard one, but I haven't done. It's still running on the batteries it came with and it's lasted really well. You set the images to rotate according to today's date and I like that function: you see exactly what your part of the sky looks like that time of year. It comes with discs for both Northern and Southern hemispheres which I also liked, in case your nephew wanted to see what it would look like on the other side of the globe! Hope this helps! There are two slide lenses that come with it. One has constellations markings and the other is the projection of a starry sky with no markings.

How big can the image be and remain in focus?

When I project it on the ceiling it's about 2-2.5 metres across. You can always manually focus the lens, so it could probably get bigger provided you have enough space (it needs at least a couple of metres between the projector and the surface you're projecting it on).
