Q: For the 12"x15" door, what does the rough wall opening need to be? From the dimension sheet I see 11.38" wide by 14.5" tall. is that correct?

yes, i would add 2/8" more in suggested opening since you can caulk it anyway

It's designed to self-close, meaning you must hold it open with one hand. Rubbish chute doors (depending on what state you reside in) need to be metal and auto-close in the event of a fire in the garage room.

Q: Is the piston adjustable so it doesn't spring back too fast?

There isn't really anything to adjust. The piston has a dampener in it that acts much like a soft close hinge on a cabinet door. The 2 that I bought close very politely.
