Does this just go between the shower head and the pipe it threads onto?

It has G1/2 thread like the shower head. It could be added between water pipe inlet G1/2 and shower hose. Too heavy to add between shower head and hose. Have a good one.

Are there head pressure requirement or limitations on inlet or outlet for this device to work optimally? For example, will 40 psi line pressure work?

Mmmm

I think there is not enough description of the product. I had to go Japanese main website to figure out the functionality. The knob need to be tight, after all. The water feel softer because I know I paid for this product. The sound of water change? It was hard to tell.

This shower head needs pressure in order to build micro bubbles as is it won't work.
