Is the medium large enough for a person and a dog?

If it's just sitting with your dog then it will be fine. If lying down with your dog it depends on how big your dog really is. Medium size: 106''L x 55''W. Large size: 118''L x 78''W.

Does it protect from bugs and the outside?

This is not a bug protecting hammock.I can wrap myself up and provide pretty good protection but it will not prevent bugs.
