Q: how can it conduct if the mouthpiece is plastic, doesnt the skin need to contact the metal sleeve?

If there's no direct human body contact with the "conductive chain" or pointer, the pointer should at least be 10" long so it generates enough "ground" by itself to make the stylus work. This is the case with both mouthsticks and headpointers. In the special situation where the pointer itself is also non-conductive - like the Kinetic Mouthstick Stylus - there's a conductive sleeve inside the hollow plastic pointer to make it work.

Q: Is the mouth piece part sturdy? The one I just bought two months ago already broke .

It is sturdier than the Joy telescoping mouthstick, however I just got it recently. So far it is holding up better than most. I really like this mouthstick and hope it holds up better than the others.

Q: Does this come with that holder?

No it does not. Holders are important and we have had to customize them for our patients. Ihsbe never found s good one online

Q: Is this compatible with iPhones?

Yeah, it works on all modern touchscreen smartphone and tablet devices.
