Q: What are the dimensions of the plate itself? Is it flat or curved like for the edge of a guitar. I want to use in a tight space to mount a 1/4" jack.

The Jack is flat although I do not know the dimensions. Being this Jack is off brand it is inferior. My local guitar shop had to replace it because of grounding issues with in the Jack.

Q: How to know what speaker wire connects to which connector on the jack? positive /negative

With only one speaker/cabinet, it doesn't matter. If you have multiple cabinets, you need to make sure all the speakers are in phase with each other. The long prong on the jack is usually positive. In the picture shown, it's the outer solder lug. Most speakers have + and - terminals. Normally, connect the + on the speaker to the long prong (tip) on the jack.
