Can this game be played in a modified way for younger children, ex: they think of a word starting with first letter on card, ignoring others?

Yes, we've played with kids 10+ and seniors up to 70 and it works well, I wouldn't push it with kids less than 10yrs.

Can this game be played in spanish? I'm not talking about the language of the instructions, i mean the game and its structure.

We are not spanish speakers, but if that is your language, I don't see why not. I purchased this game over 4 years ago and gave it to my grandchildren, I haven't played it for a long time.
