Posting is for a sheath or knife or both? I cant tell.

It depends on which item you select. Three of the items are for knives having different features for different prices. One of the items is only the sheath for $10.88 that apparently fits any of the 3 knives offered. There does not appear to be any selection for a knife with a sheath.

Is this genuine leather or plastic leather? I’ve seen both comments

It looks and feels to be a good sheath, leather yes. However it is for a specialized shaped knife, and does not fit the knife I ordered it for, my fault. "Boning Knife"

Is this available for left-hand carry?

Hi, you sparked my interest and I went to the listing and read it again. I didn’t see any information on the availability of a left hand version. The best thing I can say is that you could go back to the listing and scroll down to the question and answer section which is just above the review section and post your question. Hopefully the seller will answer you promptly. Kind regards
