Is the lunch bad insulated?

No it is not. My ice pack melted in less then 4 hours and all my food was warm.

Not that I have noticed, this set is smaller than what it appears in the picture.

Yes it does fit. You can also add a water bottle, orange, apple whatever that is small. The bag does have room to expand.

No, there is no cover on the middle box. There is one for the bottom box.
