500 CMS-1500 Claim Forms - Current HCFA 02/2012 Version"New Version"- Forms Will line up with Billing Software and Laser Compatible- 500 Sheets - 8.5'' x 11

Ordered these opposed to the fancy carbon forms we were getting locally. We use these to print from our EHR. Colors are very crisp. Paper stock is nice. Have already printed over 100 claims using them. Perfect alignment and zero paper-jams.

The 500 LOOSE sheets in a pile were put in a plastic shipping bag and sent to me. The results...lots of folded edges and crimped pages. Now dealing with submitting these non-flat forms as claims to insurance companies, especially Medicare, becomes a problem with their processing machines. The machines don't function well with folded edges and crimped pages and spit them out causing rejected claims. I paid for 500 useable sheets and can't use a bunch of them.

The print in not placed like my previously purchased forms I bought theough Amazon. I have spent over an hr trying to adjust this form on 2 different printers. The layout must be off on this print as I cannot get it right. Very frustrated.

3rd time ordering from this company ( Franklin Creative Solutions LLC) for HCFA's always on time. Great customer service. One pack came defective, a recall problem, I reached out to report it and they took care of it right away. Will order again.

After going nuts with another batch from another vendor for not aligning up after 100 claims, I ordered from this company with no problems. So thankful.
